Terms of booking

1. The service. A medical examination by a GMC-registered doctor and completion of your council's medical form or the DVLA D4 form, including an eye test. Price £45, paid in full at booking.
2. No pass guarantee. The doctor completes the form honestly from the examination and your evidence. Your council or the DVLA makes the licensing decision.
3. Your paperwork. You must bring the documents listed in your confirmation, including any specialist reports. If essential paperwork is missing, the doctor may not be able to complete the form. In that case you may rebook once, free, within 30 days.
4. Honesty. You must answer the health questions truthfully. Giving false information may be an offence and can invalidate your licence and insurance.
5. Changing or cancelling. You can change your time free, or cancel for a full refund, up to 48 hours before. Cancellations with less notice, and no-shows, are not refunded.
6. Waiting times. Some appointments take longer than planned, so clinics can run late.
7. If we change a clinic. If we move or cancel a clinic, you can choose a free new time or a full refund.
8. Completed assessments. If the assessment is carried out but the form can't be signed, the fee is not refunded.
9. Age. Appointments are for people aged 18 and over.
